Referral Program Terms & Conditions
1. Eligibility:
- a. The referring person must be an existing patient of Geelong West Smiles Dental.
- b. The friend being referred should be a new patient.
- i. Referrer needs to meet the following criteria:
- Referrer must be at least 18 years old.
- The person making the referral must have received dental treatment at Geelong West Bright Smiles Dental within the last 12 months.
2. How to Redeem:
- a. Existing patients can refer their friends by giving them a referral card (which can be obtained from reception).
- b. The friend who has been referred must bring the referral card at the time of their first appointment with Geelong West Bright Smiles Dental.
3. Thank-You Reward:
- a. The referrer will receive a reward for each successful referral.
- b. The reward will be given as credit towards dental treatment at Geelong West Bright Smiles Dental.
- c. The reward value will be determined by Geelong West Bright Smiles Dental and may be subject to change without prior notice.
- d. This reward is non-transferable and cannot be exchanged for cash (if referred patients do not show up for their appointments, the $50 credit reward will not apply).
- a. The friend being referred must be a new patient and should not have any prior relationship with Geelong West Bright Smiles Dental.
- b. The friend who has been referred must schedule and attend their first appointment for dental treatment at Geelong West Bright Smiles Dental.
- a. The referral reward will be given to the person who made the referral once the referred friend completes their first appointment with Geelong West Bright Smiles Dental.
- b. The person who made the referral will be informed about the reward, and it will be credited to their account.
- c. The person who referred a friend will be notified, and the reward will be credited to their account.
- d. This reward can be redeemed for future dental treatment at Geelong West Bright Smiles Dental and is valid for 1 year from the date of credit.
- a. Geelong West Bright Smiles Dental reserves the right to change, suspend, or terminate the referral program at any time without prior notice.
- b. Any changes to the terms and conditions will be communicated to existing patients via email, in person, or through the clinic's website.
- a. Those making referrals must comply with all applicable laws and regulations throughout the referral process.
- b. Referrers are prohibited from engaging in any fraudulent, misleading, or unethical behavior.
- c. If any violation of terms and conditions is found, Geelong West Bright Smiles Dental can disqualify the referrers and/or the referred friends.
- a. Participation in the referral program is voluntary, and patients are under no pressure to participate.
- b. These terms and conditions constitute the entire agreement between Geelong West Bright Smiles Dental and the referrers in relation to the referral program.
- c. Any misuse of the referral campaign, including but not limited to creating fake accounts, generating fake vouchers, or creating multiple accounts by the same individual, will result in the referral code being deactivated and all associated benefits being forfeited.
- d. By participating in the Geelong West Bright Smiles Dental referral program, you acknowledge and agree to the terms and conditions outlined above. We appreciate your support and look forward to continuing to provide you and your referred friends and family with excellent dental care.
- e. This referral program is not for individuals who are already benefiting from government-subsidized dental schemes, such as the Child Dental Benefits Scheme (CDBS), Department of Veterans' Affairs (DVA), Victorian Emergency Dental Scheme (VEDS), Victorian General Dental Scheme (VGDS), or Victorian Denture Scheme (VDS).
- f. Only one discount/credit can be used per appointment.
